Welcome to Abundant Community Recovery Services
SARF (Screening, Assessment, Referral and Follow-Up)
The purpose of the SARF process is not to establish the presence of, or identify a specific type of disorder, but to establish the need for an in-depth assessment. Screening is a formal process that typically is brief and occurs when a person is engaged for services.
ACRS utilizes the "No Wrong Door" approach by understanding that every point of entry into the system is an opportunity for outreach, education, and connection to needed services. This approach ensures that a person experiencing disease, resulting from co-morbid disorders, has access to a full range of services needed.
After screening and assessment, the next step is connecting Persons Served to the services they need. The assessment process is the ongoing procedure of collecting and organizing information, making immediate decisions, and formulating proper recovery and referral plans. Information collected for assessment includes: Direct Persons Served interviews, Collateral Data, and Persons Served Observations.
